Anterior Lumbar Interbody Fusion with a Mesh Cage for Spondylodiscitis after Posterior Lumbar Interbody Fusion

Abstract
Infections after spinal surgical interventions are a significant cause of morbidity. We report a case of spondylodiscitis after spinal fusion surgery in a female diabetic patient with severe osteoporosis. The patient had undergone L4-5 percutaneous vertebroplasty in our hospital 4 years ago. Eight months ago, she had undergone posterior lumbar interbody fusion and secondary dura repair surgery in another hospital before she came to our hospital complaining of severe back pain and pain in both buttocks. We applied the anterior approach to remove the abscess around the L4-5 disc space and implanted a mesh cage containing allobone. The result of a postoperative biopsy was methicillin-resistant Staphylococcus aureus infection. Postoperatively, vancomycin and sulbacillin administered for 1 month. Later, we carried out posterolateral endoscopic biopsy together with drainage and anterior aspiration for the remnant abscess. The patient's condition improved gradually. For diabetic patients, careful preoperative preparation is as important as the intraoperative procedure and postoperative care. Anterior lumbar interbody fusion with mesh cage insertion alone is an appropriate choice for some cases of infection in which pedicle screw fixation is difficult.
